Use this form to request access to an existing file share. Form must be signed by both the user and the authorized signer for the file share and turned in at the IT Help Desk .

Projects in IT are defined as temporary initiatives expected to exceed 20 hours and requiring two or more IT participants.

This form creates a review workflow for proposed high-priority IT projects at SSU. Through this form, you can provide information about the purpose and the need for the project, as well as other relevant information. This allows you to work with IT to evaluate impact and create estimates of cost, time, and resources. Only proposed projects that are authorized by your Dean/AVP/VP can be submitted using this form.

Note: If your project requires the procurement of hardware, software, or a cloud-based application please also complete an IT Purchase Review request form, even if the product is free.
