Account Requests
Managers can use this form to request access to a former employee's account data including email, files and voicemail.
Faculty and Managers (MPPs) should use this form to request a local administrative account for a state tagged PC or Macintosh computer.
Staff (non-faculty or MPP) should use this form to request a local administrative account for a state tagged PC or Macintosh computer.
Employees can use this form to request access to edit specific SSU Drupal website. New account holders will be required to complete mandatory basic training before the Drupal account is activated. Login required.
Use this form to request access to an existing file share. Form must be signed by both the user and the authorized signer for the file share and turned in at the IT Help Desk .
Use this form to request an office or departmental email account.
Enterprise Applications Requests
Use this form to request an account in Adobe Sign so you can create online forms with automation and workflow; to request assistance with converting an existing form to the Adobe Sign platform; or to request general Adobe Sign help.
Use this form for requests and issues relating to the OnBase content management system.
Use this form to request a query or a specific set of data from the MySSU and MyHR systems, to request new or updated Advisory Committees, Checklists (To-Dos), Comments, Email Communications, Service Indicators (Holds), or Student Groups, or to report an issue in MySSU, MyHR, or CFS. SSU login is required.
Use this to request new access, additional access, or troubleshooting help with PeopleSoft (MySSU, MyHR and CFS).
Service Requests
Faculty can request a Crosslist/Metacourse on Canvas from the Center for Teaching & Educational Technology.
Faculty can request a Miscellaneous or Sandbox course on Canvas from the Center for Teaching & Educational Technology.
Faculty and other Canvas Teachers can request assistance or report problems to the Center for Teaching & Educational Technology.
Students can request assistance or report problems with Canvas courses.
Employees should use this form as part of the IT Procurement process, before purchasing cloud-based products or services. This form is usually completed after the Technology Purchase Review Form.
Use this form to request new features for your SSU Drupal site. Login required.
Use this form to get faculty authorization to check out Media Equipment for class projects. Form must be signed by a faculty member and brought to the IT Help Desk at the time student checks out equipment.
Use this form to request a change in IT services that relates to an office move of 5 or more people (phone extension moves, etc). It must be filled out and emailed to helpdesk.moves@sonoma.edu.

Faculty can use this tool to quickly generate and download rosters for their courses. Also found in the Handy Tools section of Online Services.
Use this site to download free or discounted software available to SSU students and employees.
Employees can use this form to look up student and employee LDAP/Seawolf usernames.
IT Projects
This form creates a review workflow for proposed high-priority IT projects at SSU. Through this form the requester will provide information about the purpose and the need for the project, as well as other relevant information that will help the requester and IT evaluate estimates on impact, cost, time, and resources. Only proposed projects that are authorized by your Dean/AVP/VP can be submitted using this form.
Login instructions: After clicking the link to open the project request form, you'll see a "Log in to access the form" window. Select Log in with ‘Google’ and then enter your Seawolf ID.
