Use this form to request Office Email, Web or a Domain account, as well as access to an existing file share. Form must be signed by both the user and their supervisor and turned in at the IT Help Desk .
Use this form to get faculty authorization to check out Media Equipment for class projects. Form must be signed by a faculty member and brought to the IT Help Desk at the time student checks out equipment.
Please use this online form to request data from the Common Management Systems Department at SSU. After submitting your request, please allow three business days for a CMS analyst to contact you to discuss the specifics of your project.
This form creates workflow for proposed high-priority IT projects at SSU. All basic Help Desk and support requests should be routed through their regular mechanisms. Only proposed projects that are authorized by your Dean/AVP/VP can be submitted using this form.
